  1. A pop-up screen will appear, and you will need to select the Type of Incident that occurred on the on the Category Tree by clicking on the word so it is highlighted, You can click on the plus sign next to a green node which will reveal more options.

  1. You will be taken to a new page, where there will be more fields to complete. Summary Title of Incident is a free text field.

  1. Using the back to top arrow on the bottom right corner, go to the top of the screen and press Save.

  1. You will be given options to Save as Open or Save as Draft. If Saving as a draft, Alerts will not be sent.  The record is saved, however will not be visible to your Manager until this is saved as open

Section 2: How to Process Incidents. If you are continuing from adding a new Incident record, continue to step 2.

  1. If you have been alerted to an incident record you can access this by clicking a link from your email alert OR you can access through ionMy home screen Compliance > Incidents then select the record you are wanting by clicking on the View/Edit button

  2. You will now be on the Incident Detail Screen where you can review the record. When you scroll down there will be further tables for you to add additional information

    The first section on this page are your risk rating before and after controls, use the drop-down arrows to select the ratings. The rating before and rating after columns in the incident register are controlled by the likelihood and consequence that was selected on the individual record both before controls and after controls were implemented. A matrix sits behinds the likelihood and consequence fields that is used to auto calculate the rating for you.

  1. If you need to link this record to another record Scroll down the page further to “Multi Link Register” and click add new to link this record to a New or existing record, if not then proceed to step 11.

10. Enter in the closing details before closing the record.

12. Select Sign off and Close, this will then update the status to Closed. Depending on your security access level you may be present with an additional Sign Off without Closing button. This could be used if a staff member has finished their part of the investigation.  which is an easy way for Managers to see that a staff member has completed their tasks. This will be recorded in the “Sign Off” Table but the record remains open for the Manager to then Sign off and Close
